  • Project Librarian Survey (MS Word format).
       Librarians at each docMD library need to fill out this survey at email it to schnell.9@osu.edu

  • Customer Technical Survey (MS Word format).
       Librarians need to distribute, collect, and tally this survey. Email summary results to schnell.9@osu.edu or FAX them to 614-292-5757. Send orginals to Eric Schnell, Prior Health Sciences Library, The Ohio State Univeristy, 376 W. 10th Ave, Columbus, OH, 43210.

  • Customer Attitudes Survey (MS Word format).
       Librarians need to distribute, collect, and tally this survey. Email summary results to schnell.9@osu.edu or FAX them to 614-292-5757. Send orginals to Eric Schnell, Prior Health Sciences Library, The Ohio State Univeristy, 376 W. 10th Ave, Columbus, OH, 43210.

  • Customer Combined Survey (MS Word format) Both of the previous two surveys in one tool.

  • Library Turn Around Time Study (MS Word format)
       Librarians need to estimate the time it takes to send and receive ILL requests prior to implementing the docMD service. Email summary results to schnell.9@osu.edu or FAX them to 614-292-5757. Send orginals to Eric Schnell, Prior Health Sciences Library, The Ohio State Univeristy, 376 W. 10th Ave, Columbus, OH, 43210.

** The Customer Surveys should be modified to include your local information (i.e. library name, your name, mailing address, etc...)
